How to queue for free prosthetics: clarification of the Ministry of Health

Superhumans clinic of reconstructive surgery in Lviv has begun forming a queue of patients in need of prosthetics. The Ministry of Health informed how victims of the war can receive appropriate medical care free of charge.
The first Superhumans building will be fully operational this spring, however, the project is ready to start working with future patients!
In order to hand over your documents to Superhumans specialists and begin preparations for possible treatment at the center, it is enough to fill out a simple questionnaire on the Superhumans website.
The following data must be provided in the questionnaire:
- last name First name Middle name;
- date of birth;
- contact number;
- short description injury and stage of treatment;
- date of injury;
- injury medical records;
- current condition and location;
- upload injury photo ;
- the last statement of the medical history from the medical facility.
The center team will contact the author of the questionnaire and begin the necessary dialogue.
Superhumans Center – this is a modern clinic for prosthetics, reconstructive surgery, rehabilitation and psychological support for victims of the war in Lviv. Everything in the clinic will be completely free for the patient.
The clinic's supervisory board includes the First Lady of Ukraine Olena Zelenska and the Minister of Health Viktor Lyashko.
